DON’T PATCH THE PROMPT ENDLESSLY

A common mistake:
The Agent gets it wrong.
↓
Add a Rule.
↓
The Agent gets another case wrong.
↓
Add another Rule.
↓
The System Prompt keeps getting longer.
In the end the Instruction becomes a collection of patches.
Periodically do this:
Review → Consolidate Rules → Remove duplicates → Reorder the hierarchy → Rewrite the Core Instruction
A System Prompt also needs to be refactored like code.
